To make an appointment you will need to do the following:

1. Please call for an appointment

Business Hours: 9:00 AM - 4:30 PM Monday - Friday

Phone: 314-469-PAIN (7246)
Fax:     314-469-7251

Please have the following information:

  • Driver's License.
  • Insurance card and information.
  • If your insurance requires a referral, please bring the order.
  • Work injury or personal injury? Please inform our scheduler.
   
2. If your appointment is more than a week, we can mail your paper-work to your home. You may also print off the forms at this link: Forms . If at all possible, please complete our history form prior to your appointment date.
   
3. Please arrive 30 minutes prior to your scheduled time. Please note that tardy arrivals may require re-scheduling or a delay in your appointment as that may affect other patients.
   
4. Please bring the following items to your appointment:
  • Bring all medical records including X-rays, MRI/CT scans, Medical records and referral forms.
  • Bring copies of test reports.
  • If you have had previous surgery and or injections for a given problem to be seen by Dr. Suthar please obtain operative notes.
  • Family and friends are welcome to accompany the patient.
